Pelviscopy is laparoscopic surgery taken to a higher level. Sometimes the term laparoscopic surgery is used interchangeably with the term pelviscopy. Pelviscopy is the actual performance of complicated surgeries through the laparoscope. The specially designed instruments which penetrate the abdominal wall through tiny incisions are used to perform surgeries such as treatment of tubal pregnancies, removal of ovarian cysts, myomectomies (removal of fibroids), removal of scar tissue, laser surgery, and hysterectomies (the removal of the uterus) and appendectomies (removal of the appendix). The advantage of performing surgeries this way is that the patient usually can leave the hospital that same day or the next day with minimal pain or discomfort. The tiny incisions heal very quickly and are barely noticeable. Sometimes they disappear completely. The patient can then return to their normal lifestyle or to work much more quickly than if the surgery was performed in the traditional way through a larger incision. This procedure takes special training and many years of experience to perfect the techniques. Dr. Kent is highly experienced in this area.
Operative hysteroscopy is the performance of surgery through a special operating hysteroscope. An operating hysteroscope is usually larger than one used to just take a look inside the uterus. It is larger so that specially designed instruments can be guided through a channel in the side of the hysteroscope and used to do certain procedures while watching through the telescope. By using this instrument the surgeon can often do complicated surgeries inside the uterus without making incisions . Operative hysteroscopy is performed on an out-patient basis so that patients usually go home within a few hours after the surgery. It usually involves no pain. Surgeries which can be done through this instrument include removal of certain fibroids, polyps, and treatment of abnormal bleeding. This has advantages over a traditional D & C because the surgeon can see what is bleeding or what is abnormal while he is operating and therefore has a greater chance of correcting the problem.
Myomectomy is the medical term for removal of fibroids. Fibroids are benign (non-cancerous) tumors which grow in the uterus. You may have only one or two, but some people grow multiple fibroids. The uterus can grow as large as a full term pregnant uterus. Symptoms of fibroids can be multiple, or none. Some symptoms are abnormal bleeding, pain, irregular periods, increasing abdominal size, and infertility. Myomectomy is a surgical procedure which, to be done properly and without complications, needs a great deal of technical skill. Dr. Kent is highly trained in performing this surgery, using her cosmetic skills, gynecological skills and her endoscopic skills so that the uterus retains it's normal anatomical landmarks and the patient has the best outcome possible. Sometimes the fibroids can be removed by laparoscopy in an outpatient setting, or with a resectoscope through the vagina in a minimally invasive technique. Many patients choose this procedure over a hysterectomy so they may keep their uterus. Dr Kent is often sought out to give second opinions to patients whose physicians have recommended surgery or hysterectomy for this problem. Many patients require Hysterectomy for a variety of medical problems. Some patients are candidates for removal of the uterus through minimally invasive techniques such as the Laparoscope. This technique uses high tech equipment where we operate using cameras and small portals called trocars to perform the hysterectomy. The patient usually can go home the same or the next day, and only has tiny scars on her abdomen which will heal over two weeks. Dr Kent has been at the forefront of performing these surgeries.
A Resectoscope is an instrument that is directed into the uterus through the cervix (no incisions) to inspect the interior of the uterus and then perform a variety of procedures. Some of the procedures are the removal of fibroid tumors, removal of polyps, and treatment of abnormal bleeding. This operation is performed in an outpatient setting under general anesthesia. The patient can go home the same day.
Endometrial Ablation is the destruction of the endometrial lining of the uterus. The tissue which lines the uterus is called the endometrium. As women age and approach menopause, or even after menopause, they often experience abnormal bleeding. Sometimes this bleeding has no clear cause, or it can be caused by fibroid tumors, hormonal abnormalities, or polyps. The endometrial ablation destroys the lining so that the bleeding can no longer occur. Dr Kent performs multiple ablations using a variety of techniques including thermablation, cryoablation, microwave ablation and cauterization. Patients are usually delighted to have this procedure done because there are no incisions involved and they feel quite well within a few days, and the bonus is that the bleeding problems are gone.
